We apologize, but the PDF Form you requested cannot be loaded into your browser.

We detected that you are using Chrome (version 81) under Windows.
This browser does not support the advanced features needed to run our PDF Form(s).

To be able to see our PDF Form(s) you'll have to download and install the latest version of Adobe Acrobat Reader.
You can get it following this link: http://get.adobe.com/reader/

To activate the Adobe Acrobat Plug-in you'll have to type "about:plugins" in the address bar (without the quotes), and hit enter.
After that you'll see a list of all Plug-ins installed in your browser.
You'll have to disable the "Chrome PDF Viewer" and ensure that the "Adobe Acrobat" Plug-in is Enabled.

Generallly, PDF forms must be opened in Acrobat or another brand of PDF viewing software, not in a browser (which uses a plug-in).

So follow the instruction given above.

  1. Save the PDF form to your computer (probably a right-click, Save Link As).
  2. From Adobe.com, download the free Adobe Reader software http://get.adobe.com/reader/ and install it.
NOTE: you might already have a copy of the free Acrobat Reader on your computer, so look in the Windows Start Menu. You might also have a version of Acrobat Standard or Acrobat Pro, which are the full paid versions of the software. Any of them will open the form.
  3. Launch your version of Acrobat (Reader, Standard, or Pro) and open the PDF file (File / Open).

You should be able to fill in the form, save it, and send it where it needs to go.
