highlights/comments offset/moved relative to text.

Dear all,

I received back my pdf with the comments/highlights from reviewer. I can see the text bubble icon and highlight which goes like a thin band vertically across the text. Not following any text, just line going from the bubble straight up. Moreover the location of the icon itself does not correspond to the location of the comment made. 

It makes it impossible to do any ravision according to those offset highlights.

Do you have any idea why it happens? Is there a way to track back the original location?
