How to convert a 1-page document into a fillable form that can reuse again and again?

I have a 1-page document which has numerous fields. I'm looking for a simple way to convert it to a fillable form that I can reuse again and again. Is there a simple way to do this

Do you have Acrobat Professional? 

If you do, then open the document in Acrobat and select the "Prepare Form" tools. It will auto-dectect some fields and add interactive form fields automatically. Other you will need to place manually. 

Thats all there is to it 🙂 

If you want more out of the form than simple data entry, then there are endless features you can add, but that takes more work.
