How to create a PDF of PPT Notes pages

Hello,

I am trying to create a participants' document from the Notes pages of a PPT slide deck. PPT doesn't save it to the Notes page; once I create the PDF, I want to add forms for participants to enter text notes. 

Nothing has worked so far. It seems I should be able to do this in Acrobat Pro DC. Any help?

Create this page in Word, convert to a PDF page, and then insert it into the file you created from the PowerPoint Presentation.
