how to create an index of names

I need to create an index of all the names mentioned in my book.

I only have the pdf version of it. Is it possible to generate such index with Acrobat?

So far, I found no tutorials for that.

That's a very difficult task to do. First of all, how would a script know what's a name, and what isn't?
