How to duplicate form and retain unique values?

Hi there,

I have a very simple one page form that I'm trying to configure to allow for the creation of a new, identical page (infinite amount of times), but would like for the new pages to be able to retain unique values. In simpler terms, I'm basically looking to create "continuation" pages for additional text entry, but each new page needs to have the same layout as the first page.  Ideally, it'd be nice for the users that I'm designing it for to just have to click a button to create these additonal pages. I've attached the form for reference. I would be greatly appreciative of any advice anyone has to offer - I'm not very well versed with Adobe Acrobat Pro or Javascript.

Create a "Page Template".  Look it up on this forum and in the Acrobat documentation.

Hi Thom,

I had attempted that route by going to Orgnize Pages -> More-> Page Templates and creating a template off of the form, but did not see any clear avenue on how to actually insert the template to create 2nd, 3rd, ect. pages. Searching through the forum has not yielded any results that are applicable to my dilemma and the online Adobe User Guide is not very user friendly given the lack of a search function.

For some additonal context, I am using Adobe Acrobat Pro.

I am also aware that I could add this form into a Portfolio as many times as I need, but this documentation is already going to be kept in an Adobe Portfolio - so I'm not a fan of having a Portfolio within a Portfolio.

Hi,

Did you find an answer? I am at the same step as you. I have read several articles about sprawning and have inserted it into my javascript, but I am still unaware how to duplicate the page from the template.

What is the code you are using?

Also post a screenshot of the "Templates" dialog for the document you are using.
