How to embed PDF forms that are editable for users?

I have been searching for embedding PDF forms that could be fillable for user and based on user data it can calculated and can be written on its on. 

Lets step down what I needed...
1) I wan't to show User a Form where it enter its name, father name, NIC, Contact, Email, Date of Birth, Date of Appointment, Retirement Date, Address etc. (It could be simple HTML form)
2) Based on User filled form, the data is then goes to my 11 PDF forms sheet, there is a specific area for all the information, as the Name could be written in 10 times and also the other info respectively. 
3) All the data can be fetched from HTML form and embbed on these PDFs and the user just click download or print to get the data. 

What could be the best solution? If anyone related to this please guide me. If any info needed do contact me.
