Incorrect tags displayed in tagged PDF

We publish PDF from DITA XML source using RWS Tridion Docs 14.0.1 & Oxygen XML Author.  However, the tagged PDF generated Tridion does not show correct tags when viewed in the Adobe Acrobat version we use: 2024. 004. 20272. despite enabling  'Apply role mapping to tags' 

Could someone help us with identifying the cause for not being able to view the tags.

Hi Mike- Please find the attached PDF.

I don't see the tag names from the right side of your screen shot (Cover, Chapter, ...) anywhere at top level in your file, only Part entries. Also you mention role mappings but the RoleMap dictionary of the structure tree of your PDF is empty.

I might overlook something here, though, so someone who knows more about tagged files should take a look.

Hi Mike,

I've attached a PDF with custom tags. Opening this PDF with Acrobat X Pro (which is ancient), I can see the tags as shown on the right side in the screenshot shared by Kavitha. However, Kavitha has a newer version of Acrobat Pro and she does not see these tags. Role mapping was not created/applied for this PDF.
