I would like to create a form with an imput table like this using Adobe Acrobat Pro. Could anybody help?

You need to be more specific.
What exactly do you need help with?
Learning how to use Acrobat and how to create a PDF from scratch?
What are the columns 1,2,3,4 ? radio buttons or checkboxes? What do they do?
Is this table used in any way to combine values, calculate other values that are taken from other field objects such as comboxes, listboxes, date fields, text fields...?

Simply create this form in MS excel then save as "PDF". Edit saved pdf file with Acrobat then select option prepare form.
I can make it for you if you are unable to do above steps.

This kind of table is composed out to two separate elements, one is the static layout (the text, graphic elements such as the table cells, etc.), and the other are the interactive form fields. The former you can create in any application you find convenient (such as Word, Excel, InDesign, etc.) then convert it to a static PDF file. Using Acrobat you can then add the form fields on top of that layout.
Note that if you later want to edit the underlying contents you will need to go back to the original file, make the changes there and create a new PDF. You could then use the Replace Pages command in Acrobat to insert the new pages over the old ones, which will keep all of your form fields in tact. So make sure you keep a copy of the original file!
