Hello,
I am a college professor trying to use PDF forms in Adobe Reader for online assignments.
My students are running into an issue where when they try to add an image to the PDF form I made, it shows up as a black box. It works fine for me on my end, but some of them are having this issue.
Any ideas?

Thank you for sharing the file with us. We tried creating to reproduce the issue at our end and tracked the issue as below:
We opened the document with Adobe Acrobat DC (Subscription) and tried to insert the images and it worked fine. However, when we opened the document with Adobe Acrobat Reader DC (Freeware application) we observed that on inserting images using the tool it showed black box in the document.
Please note that Adobe Acrobat Reader DC is a freeware application and used for viewing, printing, signing, sharing, and annotating PDFs. We can not Edit the PDF or Insert Images into the PDF using the Acrobat Reader DC application. For more information please use the help article (https://helpx.adobe.com/in/reader/faq.html)
Please ask the users to use the Adobe Acrobat DC application (Subscription) to insert images into the PDF.

Be sure that they don't use transparent PNGs or GIFs
Acrobat don't like them.

At export of the image use a white background.
