I've created a relatively complex interactive PDF using Adobe Acrobat Pro. The form features radio buttons, text boxes, and date boxes. When I view the document and interact with it on my Mac, on which it was created, it works as expected.
However, when the document is opened in Acrobat on Windows, many radio buttons and some text boxes are not visible and can not be filled.
I've attached the file below. Any update would be much appreciated.

There are numerous form field widgets in your PDF having the Hidden flag set, e.g.
This flag is specified as "If set, do not render the annotation or allow it to interact with the user, regardless of its annotation type or whether an annotation handler is available." Thus, Acrobat for Windows here acts correctly while the program you use to view the document and interact with it on your Mac apparently operates incorrectly.
By the way, there are other issues, too. For example, both fields for the prescriber address have the same name. But fields with the same name do represent different visualizations of the same field and, therefore, share the same value. Thus, both prescriber address lines on your form always show the same value in a conformant PDF viewer...

Hey, thanks for your input. I thought I was preserving the aesthetic of my form by hiding the form field. As the fields were still interactive on my Mac I presumed this behaviour would transfer to Windows. I will fix this and your suggestion about the fields with matching field names.

You must have used a faulty application on the Mac then (let me guess: Apple Preview?), since hidden fields should be exactly that, completely hidden to the user, and they can be interacted with.

Do you use the same version of Acrobat on both systems?
