iPad PDF Form

I am trying to create a form that allows the user (iPad user using Adobe PDF Viewer) to fill in information about a reqpair request... I have successfully completed most of the form (reset field, email field, date field etc) but would like (if possible) to have a field which allows the user to insert an image from their own library (or better still, activates the iPad camera to allow the user to insert an image).

Is this possible?
