Merge / Combine fillable forma nd flat PDF files

I need to create a fillable template and merge it with a flat file, making sections of the PDF file editable/fillable. See the attached files for an example.

Is it possible to do as I explained and end up with a finished document that has the original data from the PDF along with the editable fields from the template?

 

If so, what services are used and how is it done?

You can place the "PDF File" as a background layer in the fillable file. I think that will get you to the desired combined file. You can do that using Acrobat Pro.
