Middle part of text is whited out?

Can anyone tell me why the middle part of my text looks like it has a box through the middle of it and it is cutting out part of my text?  I have attached a screen shot below, you can see the bottom of the text cut off on the first line.  I have attached form as well.

Thanks for attaching the form, as it was easy to check what was precisely causing trouble here.

So, when we opened the PDF form, it had an additional rectangle box added over the first line.

Removing the rectangle text box solves the problem.
