Multiple filled pdf forms converted to spreadsheet

I have about 100+ emergency card forms that i would like merged into a spreadsheet.  I have tried to use Acrobate to do this but the form fields are not all being pulled consistently.  Why is this happening?  All the forms are identical.

The form is useful, but not enough to explain the problem. I think it might be best if you gave an example of the "not being pulled consistently" results and how you set up Acrobat. (Using test data, as you have).
