It has been resolved and spent a day and half! Wish the Adobe API has more capabilities. So far such tasks are all work around ...:-)

At the options of list box you can enable "Multiple selection".

On a form yes, but you're screen shot is of a dialog. What exactly do you want?

Hi Thom,

Yes, the screenshot is a dialog box, but it seems to me that Adobe doesn't have such dialog box. If it can use exist Adobe controls, it is also fine. Thanks a lot, if you have those Java scripts to share. -G

You can have a popup dialog in a PDF form:

ACRODIALOGS OVERVIEW

But the list dialog element is not multi-select.

So perhaps you are better off using form fields.
