I need to create a Form in PDF.
That Form will have a Checklist - just an example below:
1) Name - to be filled by the recipient - - - - - Upload Tab for uploading an ID proof
2) Do you drive? - to be filled by the recipient - Upload Tab for Driving License.
So on and so forth a list of questions beside which there is an Upload option wherein recipients upload files/documents.
Result: I receive the Forms filled with the data plus the files attached.
In what format: Such that the attachments are downloadable together with the Page 1 Checklist in a SINGLE PDF. the recipient can too download the single PDF document so made after entering and uploading the documents.
How is this possible to do?
