Not able to upload image in pdf form

Hi, I have adobe acrobat DC and have a form which I need to upload images. When I click the box, a widow is supposed to open providing me a browse option to upload the image. There is no window opening even after I tried multiple times. The same pdf is opening in my colleauge's pc. What could be the problem?

I've removed the password.
In fact, the fields on the last page are just buttons, so you need to add this script to them in Action: Mouse up

event.target.buttonImportIcon();
