So I encountered this problem too, but playing around with a couple different things I fixed it.
I had the edge of a placed pdf hanging on the edge of my page.  

Once I pushed my crop frame out of my page all the other images have alternate text errors went away.

The other pdf was tagged so this shouldn't have happened.  I also realized I was getting errors for all of the graphics nested inside that pdf.  I don't get it but it worked.

I’m curious what the source application was that created this PDF file. Regardless, this error is typically indicating an untagged object that Acrobat things is an image (and most likely is) within the document. Acrobat isn’t always good at showing what object it’s referring to so it can be a bit tricky to find. If you open the Content pane in Acrobat, look for any objects without a container. These would be untagged objects. Select the object and then right click and choose “Create Artifact”. Or if it’s a legitimate element, you can tag it as such. When you highlight the object in the Content pane, you’ll see it highlighted in the document so you can identify what it is.

Hi Chad Chelius, I created a bi-fold brochure in InDesign and did as much 508 in the source document as I could before exporting it as an interactive PDF. I had one error that's stumped me (see screenshots). I included alt text in Indesign yet a few elements seem to be acting up. Did you find a solution to this? I have the same issue. Exporting from the latest InDesign 19.4x64 to Acrobat Pro PDF. Have never had an issue before but now the article tags are only exporting some photos with tags and not others. All photos are setup the same way, all are setup with the alt text selected in export options. However there are 6 photos that get moved to the "Other elements alternate text - Failed" section in the acrobat accessibility report, as per your same screenshots below. And then in the tags panel in Acrobat, only half of the tagged articles in InDesign have transferred through as tags in the Acrobat tags panel. This has never been an issue before, and I have not changed my accessibility processes. The only difference is this is a different client and different document I'm working on.

I have sinced solved this annoying error in the same way. I had photos bleeding across to the opposite page, so I had to separate them into a box for each page. I also realised it would not accept images that fell on the centre spread. If I moved them 0.25mm off the centre spread it no longer showed an error on the opposite page.
