Overlapping text on pdf form

I'm looking to see if anyone has a fix on overlapping text when someone is filling out a form created on adobe acrobat. I initially created the document in word and saved it as a pdf and then used the prepare form tool on adobe.   TIA

Did you mean to say PDF form field objects not properly aligned with the static text content ?

 

I also don't see any overlapping text content.

The only thing that looks a bit like overlapping text is item #2 under Attachments. If that's what you're referring to my guess is there are two fields with the same name one on top of another. Click the top one (in Prepare Form mode in Acrobat) and hit Delete. If there's another field underneath it, that's your problem.
