Prepare form missing E-signature and E-signature block filed

I'm pulling my hair. I've two PDF file. When i open first PDF it's missing E-signature, E-signature block filed, Initials filed and so on.

 

1 st PDF

2 PDF have everything. How do i get all the fields in 1st PDF.

The first is a Acrobat form. The second is a Adobe Sign form.

You change the form with the menu at ...

So, you are using the new Acrobat UI.  It has most of the same options as the old Acrobat UI, but in different places and sometimes with different names. 

In "Prepare Form" mode the option you want is at the top of the "..." menu just to the right of the "Prepare Form" text in the top left corner of the screenshot. 

 

If you would prefer the old Acrobat UI, like most people, then see this screen shot.
