Prepare Form Tool Not Working

Hello,

The Prepare Form previously worked, so that all fields could be automatically detected. Nothing has changed with my forms, yet when I go to recognize the fields, Adobe does not recognize most of the form fields that it previously populated.

There has been no change in the fields I use. Why is it not working now?

Does it recognize some or none?
