Prepare Forms not working in Acrobat

I am trying to use the Prepare Forms tool in Acrobat to allow myself and others to fill in a document. The tool will seem like it is working and let me fill in the document, but as soon as I click preview or close the tool, everything disappears. The only way to see the information is to open Prepare Forms again. How do I get the fields to actually save to the document?

The fields should still be there. If you don't see them enable the fields highlighting option, by ticking the box next to "Show border hover color for fields", under Edit - Preferences - Forms.
