Preparing a Form - multi-line Text Field, non-rectangular

Is it possible to have an unevenly shaped text field? I'm preparing a form into a fillable PDF. The form is from an Insurance company so I can't edit the overall structure. They have a place to type multiple lines for the same field, but the prompt takes up a portion of the first line. So if I want to have that be one text field, I need a smaller text field on the first line and a larger one on the second line. But I'd like to make it one field if possible. 

I've provided an image of the portion in question below. I would like those two text boxes on separate lines to be as one field. If that's even possible.

No, you can't have single field shaped like that, but you can limit the number of characters in the first field and have it auto-tab to the second field.  This might split up words without a hyphen though:
