Printing a PDF issue

When printing a PDF in "actual size" the alignment is complete out, I have tried printing as Image and that hasn't worked. I need to print as actual size due to it being a sewing pattern. the print preview shows it perfect.

I have a Mac and Epson ET-2118 both on the latest operating systems.
