Printing on 9 1/6" x 5" preprinted cards -- Form Fields not lining up

I am using pre-printed cards that are an odd size, 9 1/16th" x 5".  I scanned the card with my Fugi Scansnap and brought it up in Adobe.  I created all my necessary form fields and they line up well on the scanned copy. When I choose to then print the form fields only, if I use a standard size sheet of paper to test, it looks good and lines up with the card if I compare.  However, when I use the actual card, everything on the card prints about half an inch to the left of where it needs to print.  I'm sure it has to do with the card size or something.  If I use the feed tray on my printer and leave it as if it were a standard size sheet of paper and place the smaller card all the way to the left, it prints okay, but it feeds funny and it's a little tilted, so some fields get skewed too high.  I feel that there has to be an easy way to fix this.  Any ideas?  Thanks.
