Problem duplicating button to all pages

I've been adding a "back to index" button to documents and then duplicating to all pages.  Now when I try to duplicate the button it only duplicates on the very last page and leaves the ones in the middle (343 pages) with no button.  I ended up with a dozen buttons on the last page.  What can I do?

How did you duplicate it, exactly? What application (including exact version number) are you using?

I'm using Adobe X Pro V10.1.16.
I have managed to resolve the issue by leaving the button in place and
creating a second one, which duplicated successfully. I then deleted the
first one.

A bit of a work-around, but it worked this time.
