Problem with Numbers on a Form

I'm currently trying to fill out a form from a former employer.

Everytime I put in numbers (ID number, phone number, date, etc), the those boxes all turn into the same number. For example, when I try to put in the date 03/01/2022, all the numbers become a 0. I try to add the 3 after the first 0, all the boxes become 3.

I hope this makes sense and maybe someone has a solution that can help.

You must give each field a unique name. Also, it's not a good idea to have a separate field for each number...
