Problems tagging and removing tags

I have been having some problems tagging items in Adobe Pro DC that I was hoping to get some help on. For some reason, I am often unable to tag certain content in my PDF. No matter how much I select an item and assign it a tag, the item will not appear in tags panel or reading order. For example, I have been trying to tag a paragraph as text and no matter how much I highlight the paragraph and select the paragraph tag, the paragraph will not get tagged. I have even tried to tag it as something else to simply assign it a tag (such as a heading or image) and the item will still not get tagged.

In addition to this, I have also been experiencing the opposite issue where items that I want to remove the tag from (e.g. assign them as background content) still remain tagged.  For example, I have been trying to remove a tag from a decorative line in my document and the line will still remain tagged.

So far, the only way I have been able to “fix” both these problems is by clearing my entire tag structure and starting all over again. Has anyone else encountered this? If so, have you found an alternate solution other than clearing the tags and restarting again?

Usually - not always - when this happens to me I can fix it by going to the Content pane, find the content element that resists tagging, and drag that element out of whatever container it is in. It usually can then be tagged. Same approach goes for artifacts. Always save a copy of the document first just in case something goes horribly wrong, and be cautious if you encounter Form XObjects. Dragging content out or otherwise messing with Form XObjects can really screw up a document. I hate those things.

I'm new at this and have the same issues.  How/where do I find the container where the element resides.
