"Child" Bookmarks Default to Expanded

I have documents with "child" bookmarks under a "parent" bookmark (e.g. a 4-page pdf, with the bookmarks for the last 3 pages indented under the title page). However, no matter how I configure the setings, or how I save the file, it always opens with the bookmarks collapsed. How do I force the document to open with all (or some) bookmarks expanded?

You can use JavaScript to open the bookmarks.

var bkm = this.bookmarkRoot;
if (bkm.children != null) {
    for (var i = 0; i < bkm.children.length; i++) {
        bkm.children[i].open = true;
    }
}

 However, this may not work in Reader but Acrobat.
