"Document Properties" Dialog Box when opening a specific pdf

I have created some java script in the pdf to automatically put "today" in a date field. Now when I open the pdf, it shows the date field with today's date but the pdf also opens the dialog box for "Document Properties". Can someone tell me what I've done wrong? I have never had that happen before but this is my first time using javascript in a pdf as well. maybe an extra space somewhere? Please help?

That's very very strange. I don't think JS is even capable of doing that... Can you post a screenshot of what you're seeing, or better yet, share the actual file with us?

Hello again,
Here is the form. So when you open it, you get the “Document Properties” dialog box. Maybe there’s a setting in the form that has been changed because no other PDF does that to me. Just this one now that I’ve done these things to it.

To share a file upload it to something like Dropbox, Google Drive, Adobe Cloud, etc. and then post the link to it.

Can you share the file?
What application (including exact version number) are you opening the file with?

I have no way to share the file but with the way I did. I don’t have drop box or government online accounts. I’m running adobe DC 2015. I could create a dropbox account for myself.

It’s not available to me. It’s a government version and track and send is not an option. If I were doing this privately….

Then I'm afraid I can't help you further, but I don't think it has anything to do with the script.
Try restarting your computer. If that doesn't help try running a Repair Installation of Acrobat.
