As of today, our team is not able to use the "Fill in form fields" button that is normally found in the toolbar. When clicking on Customize Toolbar is still shows that it is selected but there are no options to update it from the options. The Fill & Sign is also not working so we have no access to add to a form for comments or other text updates after the form has been filled out. I tried saving as Save As Other > Reader Extended PDF > Enable More Tools (includes form fill-in & save) option and it did not make a difference.

Did you maybe sign the file? Or otherwise protected it?
And the Extended Reader Rights are no longer needed. In fact, they can cause issues, so it's best to avoid applying them.

I do not sign the forms we work with. If there is a signature, how would I know and can I remove it? Thanks.

It should appear in the Signatures panel. Right-clicking it will allow you to clear it, if you applied it.

There were no signatures showing.
I just created a new form by Printing to Adobe PDF and saved it to a new location on my computer. I opened it and I see the "Fill in Form Fields" button is showing in the toolbar. I went to Prepare a Form and after it was done creating the form the "Fill in Form Fields" button was gone again. Is this feature now only available if I don't use the Prepare a Form? I.e. the PDF can't already be formatted as a form? Thanks.

I don't understand what this button is supposed to do. I never encountered it before myself...
You don't to click any button to fill in form fields. You just click into them and fill them in directly.
