Hi, for our school I am designing the report ('rapport') for the students. We want a selection menu where you can choose 1, 2, 3 or 4 dots to indicate how good a student is doing.
I've added what we have so far. But it is in Word, and the selection menu is not user friendly (not all teachers are adequate with pc's :)). Is there a way to make it in PDF? I have only found the check boxes and the radio button, but I can't get both to work as a selection where you can check more than 1 box.....

Create one checkbox and set it as you need.
Use the More menu to "Create multiple copies".
Move the 2nd column (using the Shift key) to fit the layout.
