When I open the PDF document through files, there's one group of two radio buttons that aren't working. All of them work fine when I'm in Adobe Acrobat, but it's only when I save and then try to open the file that these two buttons aren't working. They all have different groups, for example, question 7 is in group49 and question 10 is in group 50. The two that aren't working are in group58 and I've made sure that's the only group58. The specific question I'm talking about is on page 4, question 10 (b.).

I recreated the scenario in two different computing devices.
I downloaded and opened your shared file in both an Android mobile device and a laptop with a full desktop version of Adobe Acrobat on Windows 11.
The results:
The PDF form is working fine for me with the full desktop version on the laptop computer, but I have to admit that I truly didn't understand

I have seen a lot that check boxes are usually the way to go when having a document like this. This is just something I'm doing on the side so I'm not able to make any big changes like that unfortunately. I was able to get the radio buttons to work for question 10 (b.). I believe saving it onto my desktop and using a different file name helped because it is shared with other users over a network. I also just cut the previous buttons and inserted new

The PDF form is working fine for me with the full desktop version on the laptop computer, but I have to admit that I truly didn't understand the logic behind grouped pairs of radio buttons that are NOT listed in a numerical sequence.
And I couldn't help but notice that the export values that you've assigned to each radion button corresponds to other questions in the document.
It also confused me to see radio buttons using the same export values of Choice1 , Choice3, Choice5, for example, which are all mixed up with the same export value that is used by many other radio buttons listed under column Yes or column NA.
Unless you're planning on adding additional field objects to perform calculations (which will grab the assigned export values of any radio buttons that were marked as "Yes"), this may not be the bproper way of employing export values with grouped pairs of radio buttons (but this is just a personal observation, it may have nothing to do with the root of the issue).
But in any case, for each radio button that falls under the "Yes" column, I would've assigned an export value of "YES". And for all the radio buttons that fall under the "NA" column I would've assigned an export value of "NA".
Additionally, if this form will be re-used many times among different users, then it is missing a reset button to clear all the text fields and reset all radio buttons to its default state (all radio buttons under the NA column should be marked after a reset).
For instance, the next user who will use the same form should be able to reset the form fields values to its default state.
On a separate note, It is worth observing that when I used Save As a Copy from the Acrobat Mobile app, it throws the behavior that you are describing.
The discrepancy triggers between the grouped pair of radio buttons assigned for question 16b on page two and the grouped pair of radio buttons assigned to question 10b on page 4 (as you've described).
It appears to affect each other after employing Save As Copy and opening the new copy (even when each group of paired radio buttons have their own unique radio button group name) ... which is very strange.
I assume that this is the discrepancy you're referring to??
Which makes me wonder if the reason to why this is happening is because the file that you are sharing is already being shared simultaneously with other user(s) over a network ???
Please clarify.
If my suspicion would be correct, this may be resolved by saving a copy of that shared file with a different file name outside of the shared network storage location or simply by saving it locally on the desktop computer.
