Readable PDF files

I'm trying to recreate the attached screen grab in Acrobat for my clients website. It's a 16 page A5 booklet that was recently printed  the header bar info has me puzzled - I don't even know what you'd call this PDF -interactive or readable, consequently, I can't find a tutorial for it.
Please help. I'm assuming this can be exported from Acrobat  and is not some 3rd party software

With the limited information available not sure what this can be.

If you want to screen grab and convert the file to PDF that can be done.
