Assuming you're using Acrobat, you'll want to activate the Forms tools. Once the Forms toolbar pops up, you can use the Button tool & draw a rectangle with it. Give the button a name, and click on the All Properties link in the yellow box. Go to the Actions Tab, and set the Mouse Up Trigger & Reset a Form as the Action, and then click Add. You can specify what fields are to be cleared in the next dialog, and hit OK to finish.
