Revert From Saved Form as Read Only

I created a form in Acrobat Pro DC and then accidentally saved a copy as read only after adding my signature. The file overwrote my form and now I only have a read only copy that has contents in every field that I cannot remove. How can I get my blank form back?

Right-click the signature field and select Clear Signature.
