the pdf is written in RTL language , but there are some expressions in english (LTR),
when a screen reader reads a RTL and LTR text in the same paragraph, it messes up the reading .
what can be done?

I don't have personal experience with this, but this is what I would try .... Set the document language property to the appropriate RTL language. Nest each LTR expression in a <Span> tag and set the language property of each <Span> tag to the appropriate LTR language.

Can you please let us know if the suggestions made by a_C_student worked for you?
I had an input some time ago about tagging a PDF with Arabic content PDF (RTL) which may or may not address the issue you're facing, see if helps you out.

no it is not the correct answer for me, but perhaps it will help others,
In my case , the problem is that software's like older versions of Microsoft WORD, the pdf converter, that is used to create this PDF , messes up blocks of mixed rtl ltr text.
so it doesn't relates to Acrobat TAGGING at all , but rather to other software where the text it self that is converted comes out all garbled up.
