Script for piping text field data into email subject line

Hello,

I'm using Acrobate Pro. Can anyone share script which pipes in user inputted data from a text and/or date field from a form so that it's included in the subject line of an email. I'm using the Submit Form Action to allow users to submit a form via email once completed.
