Single Template with Changing Document Name

Currently trying to figure out the best option for creating NDAs, formal contracts, and fillable forms for my company. One of the forms is an intake form, so I created a form template, but then it shows up with the same title and I cannot easily find a specific client's document. If I create a signature template then I need to have a signature on a simple form which is not what I need. Same issue with NDAs. I need to create a template with form fields, no signature, and the ability to change file names in order to track the client and document type. Having background info like when it was viewed and signed is, of course, important as well.

 

Is there an easy fix for this? I have obviously not attempted any custom coding, but if that's what it takes to make this process as easy as it should be, I'm up for it. I have very beginner knowledge of any programming languages but I'm resourceful, so any help is appreciated.
