Tables formating in PDF

Hi! I am new to Adobe Acrobat. How can I create this type of form with table? Where is the option to insert a table and its format?

If your PDF file already has the visible grid of the table (like what we see in your screen capture), then you just add the form fields individually and superimpose them on the table grid.

 

If the grid isn't in the PDF, then there is no way to add a table to a PDF and then add the form fields to the table cells.

Return to either MS Word or Adobe InDesign where you can create the table, and export a new PDF with the table grid.

 

Also, InDesign can not only create the table but also add the form fields which then can be exported with the form fields already done. But InDesign is a professional quality program and requires substantial training to use.
