I've been having an issue where if I fill in a text box, it fills in another text box somewhere else. This will happen on several forms and it has only been happening recently in the past few weeks.
I have attached a form that this is specifically happening.
Page 2's 'Other (Specify)' box and page 6's 'Date Signed' middle day text box are essentially linked, whatever you enter in those boxes will show up with the same text.
I originally opened it on a computer with win10 with adobe acrobat pro where I noticed the problem. I then opened it on 3 other win10 pcs with adobe reader to check, same thing. The odd thing is that this same file, when downloaded and opened on a macbook with adobe reader works perfectly fine and it doesn't happen.
Is this any settings issue that I can change or version change? This is frustrating with adobe.

One of the fields was copied/pasted from the other. This links them with the same name but differenitated by a #0, #1, #2 etc. They will be linked until you give them unique names.

I have this exact problem! I filled out an Advance Directive form in large type, then copied each answer to a new form in standard size type. In one area only, when I type in box 1, the text appears in box 2 etc as described by OP. How do I clear this linkage, e.g. where/what do I rename so that I can have different text in the different boxes? It only happened in this one place? Hopeful to hear from someone soon, I know this is an older post.

In this example I did one text box and then copy pasted that box. You will see that in Prepare Form the fields have the exact same name. If I change the value in one box it will change the other. To fix it, In acrobat pro you go into "Prepare Form" to edit the fields. On the right hand side rename the second field to something other than "Text1" (the #0 and #1 indicate a copied field of the same name).

I don't have Acrobat Pro, just the free reader app. Using a Mac, the form I got from Kaiser always locked, so I downloaded Reader in order to be able to fill it in.
