Hello.
The images in the PDF are being cut to pieces when the file is saved. Any idea how that happens and how to avoid it?
The Preflight > Detect and merge image fragments option doesn't help to stick the pieces back together.

This is a normal consequence of the flattening of transparency.
There is nothing you can do against it, except not to flatten the transparency, i.e. you have to export to PDF 1.5 or later version.

Save the original file and make a copy, by saving your copy as encapsulated postscript file and open it again. See if that allows you to continue to use the preflight tool option that you are describing.
On the other , when I look at the screenshot you've provided, those frames look more like text boxes, or text frames rather than image frames.
If you are able to edit the text in those boxes and type anything in there is not an image frame.
It almost look like the open document is still in preview mode and you did not fully closed the edit mode before you used the preflight tool (but I may be entirely wrong)...
