The "Submit" button does not work

Need collect the form data as attachments by e-mail

Everything was done according to instructions from here

When you click on the button, nothing happens

I have the same issue.... only receipients with full Adobe acrobat can hit the submit button.  Those with Adobe Reader hit the button but nothing happens.   I find this feature pretty much useless as not everyone has the full program.
