Three Signatures in one PDF

I have a pdf form with spaces for three signatures.  I have gone through fill and sign to request signatures (this did not work).  I am able to asign one signature field to two people.  When I add the third, I need to change the "date" field that autopopulates to "signature".  That person was good to go.  However, after that, the previous field that were set one signature field per person, now try to have two signature field for one person.  All I need are three boxes with three different signatures on one form.  I have Adobe Acrobat Pro.  Help?

Could you please confirm how you created the PDF form? It would be helpful if you could share the PDF with us.

You may add the form fields to the document after you use the Request Signature feature.

This was fixed by changing the order the fields were filled in.  I had to select the third box that autopopulated at "date" as the second signature, then I could label my third signature.  A bit backwards, but it worked!
