Use a PDF to build a submittal package

Hello all-

I'd like to see if it is possible to do the following:

A user lands on a specific web page to fill out a PDF. On page 2 there are ~9-12 check boxes/radio buttons for the user to select certain documents to add to their submittal bundle.

If the user selects 1, 2, or however many documents in the form, in the background the sheet grabs the PDF's from a folder on our website and creates a 'bundle' with the exact documents they want. Then they would download the new customized PDF bundle with the cover pages.

Is this possible? I could swear I have seen something like this once upon a time but can't for the life of me remember..

Yes, it's possible to implement that functionality. You will need a good web programmer, and it will work. It's, however, a web problem, not a PDF problem.
