Using multiple groups of check boxes in Adobe PRO DC

I am new to creating fillable  forms in Adobe Pro and am trying to create several yes/no fields on an application.  No matter how I try to create check boxes for each group, Adobe will not capture all of them (some boxes stick and some don't).  I have tried giving each a unique name, but to no avail.  Any pointers are greatly appreciated. Thank you.

As per the issue description mentioned above, you want to create multiple groups of checkboxes on a pdf using acrobat dc, is that correct?
