When inserting a PDF with auto-filling form fields, some fields disappear?

Good evening, I am having a huge problem, where the form fields in our PDFs are disappearing, when we import them into our blank PDFs.

Any help would be greatly appreciated!

What do you mean by "inserting"? Are you inserting pages containing form fields from one PDF into another PDF that contains form fields? If this is true, then field values may be overwritten if the two PDFs contain fields with the same names.  You do know that fields with the same name are essentially the same field, and so they share the same value and the same scripts.

Also explain what you mean by "auto-filling".  Merging forms will activate the calculation scripts.

We have found that a lab we work with is using a certain setting, with incompatible software, which has been messing up our forms when we receive the PDFs back from them.  Sorry for the inconvenience!  gauravd83019631
