why are the pages that I have combined look like this

How were the PDFs generated?
What app is being used to combine them?

Hi Bob, 

thanks for the questions and forgive me,  I'm very new at this and never had an issue before.  I am using Adobe Acrobat Pro.  I am combining diffent document into one after I have filled them out and I get the difference in size.
