Why is my document secured?

My pdf has no password protection. I created it from scanned pages. I get a message that I cannot edit because it is a secured document.  The security tab in properties already says no protection.

What program produced this PDF form?

Is this a government form?

I produced it using Acrobat DC.  I scanned several pages from each of two books (not copyrighted).  It is not a govenment form.

Did you sign it? Or use anything related to "Reader extensions"?
